Description

Clarithromycin is a powerful macrolide antibiotic that works by inhibiting bacterial protein synthesis, thereby preventing the growth and spread of infections. It is effective against a wide range of bacterial infections and is sometimes used in combination with other medicines to treat stomach ulcers caused by Helicobacter pylori. Clarithromycin also exhibits mild anti-inflammatory properties, making it beneficial in certain chronic respiratory infections. To ensure complete resolution of infection and avoid resistance, the full prescribed course should always be completed, even if symptoms improve earlier.

Uses

Clarithromycin is indicated for the treatment of:

  • Respiratory tract infections (pneumonia, bronchitis)

  • Sinusitis and pharyngitis/tonsillitis

  • Skin and soft tissue infections

  • Helicobacter pylori in peptic ulcer disease

  • Atypical bacterial infections

  • Lyme disease

  • Prevention of infections in immunocompromised patients

Dosage

Dosage varies according to age, weight, and type of infection. Common regimens include:

  • Acute sinusitis: 250mg daily for 5 days

  • Otitis media: 7.5mg/kg (max 250mg) for 5 days

  • Pharyngitis/Tonsillitis: 250mg daily for 5 days

  • Chronic bronchitis exacerbations: 250mg daily for 5 days

  • Skin infections: 15mg/kg (max 1000mg) daily for 14–21 days

  • Pediatric use (drops): Dose adjusted according to weight, always under physician’s advice

In Case of Overdose

Overdose is usually not life-threatening but may cause abdominal pain, nausea, vomiting, or diarrhea. Medical evaluation is essential. Stomach wash, activated charcoal, and monitoring may be required in clinical settings.

Missed Dose

If a dose is missed, take it as soon as possible. If it is close to the next dose, skip the missed one and continue the regular schedule. Do not double the dose.

How to Use

Shake the bottle well before use. Measure the dose with a proper dropper or dosing device rather than household spoons. Take with or without food, as advised by your doctor. Always complete the prescribed course.

When Not to Use

Clarithromycin should be avoided in the following conditions:

  • Known allergy to Clarithromycin or other macrolides (e.g., azithromycin, erythromycin)

  • Pre-existing heart rhythm disorders

  • Severe liver or kidney impairment

  • During pregnancy and breastfeeding unless specifically approved by a doctor

  • Concurrent alcohol use, which may increase stomach irritation

Side Effects

  • Elevated plasma CPK levels

  • Rash, urticaria, angioedema, erythema multiforme

  • Rare kidney-related effects (nephritis, nephrotic syndrome)

  • Vertigo, dizziness, hearing disturbances

Storage/Disposal

Store at room temperature in a cool, dry place, away from heat and direct light. Keep tightly closed and out of reach of children. Discard any unused or expired medication as per pharmacist’s instructions—do not flush in toilets.

Laboratory Screening

Routine lab monitoring is usually not required, but in prolonged therapy or in patients with pre-existing conditions, liver and kidney function tests may be advised. ECG monitoring may also be recommended in patients at risk of arrhythmias.
