Description

Amis 100mg tablet contains amisulpride, an atypical antipsychotic from the benzamide group. Made by Rite Pharma. Pack of 10 tablets. Prescription only, dispensed under psychiatric care.

Uses

Amisulpride blocks dopamine D2 and D3 receptors. At low doses it mainly lifts the flat, withdrawn side of schizophrenia, at higher doses it controls the active symptoms. It is prescribed for:

  • Acute schizophrenia: hallucinations, delusions, disordered thinking.
  • Chronic schizophrenia: ongoing maintenance to stop relapse.
  • Negative symptoms: lack of motivation, social withdrawal, emotional flatness, usually at the 50 to 300mg range.

It is sometimes used off label for dysthymia and for augmenting other antipsychotics, only under a psychiatrist.

Dosage

Set by the psychiatrist and adjusted to response. Standard adult ranges:

  • Negative symptoms mainly: 50 to 300mg once daily.
  • Acute psychosis: 400 to 800mg daily, split into two doses.
  • Maximum: 1200mg daily.
  • Doses up to 300mg: can be taken once a day. Above that, morning and evening.
  • Kidney patients: dose cut by half if creatinine clearance is 30 to 60, by two thirds if 10 to 30.
  • Elderly: lower doses, higher risk of drowsiness and low blood pressure.

How To Take

  • Swallow whole with water, with or without food.
  • Same time each day.
  • Missed dose: take it when remembered unless the next one is close, never double up.
  • Do not stop suddenly. Symptoms can return fast and withdrawal effects like nausea, sweating, and insomnia can appear. The doctor tapers it down.
  • Full effect on psychotic symptoms takes 2 to 6 weeks, keep taking it even if nothing feels different in the first days.

Side Effects

Common:

  • Drowsiness or, in some people, trouble sleeping.
  • Restlessness, feeling unable to sit still.
  • Tremor, stiffness, slowed movement.
  • Dry mouth, constipation.
  • Weight gain.
  • Dizziness on standing.
  • Raised prolactin: stopped or irregular periods, milk discharge, breast enlargement in men, low sex drive, erection problems.

Less common: blurred vision, low blood pressure, raised blood sugar, seizures in people already prone.

Stop and get medical help for:

  • High fever with muscle rigidity, confusion, sweating, fast heartbeat (neuroleptic malignant syndrome).
  • Fainting, palpitations, or irregular heartbeat (QT prolongation).
  • Repetitive uncontrolled movements of the face, tongue, or limbs (tardive dyskinesia, can become permanent).
  • Sore throat with fever and mouth ulcers (low white cells).
  • Pain or swelling in one leg, sudden chest pain (blood clot).
  • Swelling of face or throat, severe rash.

Precautions

  • Heart: avoid with slow heart rate, known QT prolongation, or low potassium and magnesium. A baseline ECG is usual before starting.
  • Parkinson’s disease: worsens movement symptoms.
  • Epilepsy: lowers seizure threshold.
  • Diabetes: blood sugar can rise, check it.
  • Elderly with dementia: raised risk of stroke and death, not recommended.
  • Driving: do not drive or run machinery until you know how it affects you.
  • Alcohol: avoid, it multiplies the drowsiness and dizziness.
  • Pregnancy: only if the psychiatrist decides the benefit outweighs the risk. Babies exposed in the last trimester can show withdrawal or movement problems after birth.
  • Breastfeeding: not recommended, passes into milk.

Contraindications

  • Prolactin dependent tumours (breast cancer, pituitary prolactinoma).
  • Pheochromocytoma.
  • Children before puberty.
  • Combined with levodopa.
  • Combined with drugs that prolong QT (see below).

Interactions

  • QT prolonging drugs: avoid with sotalol, amiodarone, quinidine, erythromycin IV, haloperidol, pimozide, thioridazine, methadone, and some antimalarials.
  • Levodopa: the two cancel each other out.
  • Sedatives, opioids, antihistamines, benzodiazepines: stacked drowsiness.
  • Blood pressure medicines: extra drop in pressure.
  • Diuretics: low potassium raises heart risk, potassium needs monitoring.
  • Clozapine: amisulpride levels can rise.

Storage

Keep in the original strip below 30°C, away from moisture and sunlight. Out of reach of children and pets.
