Volvez 100MG Tablet

Volvez 100mg Tab contains cefpodoxime proxetil, a third generation cephalosporin antibiotic. Made by Opal Laboratories (Pvt.) Ltd., Karachi. Pack of 10 tablets. Prescription only.

Uses

Cefpodoxime treats mild to moderate bacterial infections. Doctors usually turn to it when a simpler antibiotic has failed, when the patient reacts to penicillin, or when a culture report shows the bacteria are sensitive to it.

• Tonsillitis and pharyngitis.
• Middle ear infection (otitis media).
• Sinusitis lasting over 10 days.
• Acute bronchitis flare-ups and community acquired pneumonia.
• Bladder infection (cystitis) in women.
• Skin and wound infections that are not severe.
• Gonorrhoea, single dose.

It has no effect on viral illness such as cold, flu, or dengue.

Dosage

Follow the doctor’s prescription. Standard adult ranges:

• Throat, tonsils, bladder: 100mg every 12 hours, 5 to 10 days.
• Sinus, bronchitis: 200mg every 12 hours, 10 days.
• Pneumonia: 200mg every 12 hours, 14 days.
• Skin infection: 400mg every 12 hours, 7 to 14 days.
• Gonorrhoea: 200mg once.

Kidney patients (creatinine clearance below 30) take the same dose once every 24 hours. Dialysis patients take it three times a week after dialysis. No change needed for liver problems.

Children under 12 usually get the suspension, not the tablet.

How To Take

• Take with food, absorption is much better after a meal.
• Swallow whole, do not crush or chew.
• Keep doses 12 hours apart, for example 8am and 8pm.
• Keep antacids and acid reducers (Gaviscon, omeprazole, famotidine) at least 2 hours away from the tablet, they cut absorption.
• Missed dose: take it when remembered unless the next one is close, never double up.
• Finish the full course even if you feel better in two or three days.

Side Effects

Common, mostly mild:

• Diarrhoea or loose stools.
• Nausea, stomach ache.
• Headache.
• Thrush (oral or vaginal) after longer courses.

Stop and get medical help if you notice:

• Swelling of face, lips, or throat, wheezing, trouble breathing.
• Blistering or peeling rash.
• Watery or bloody diarrhoea with cramps and fever, during or up to two months after the course. This can be C. difficile infection. Do not take Imodium for it.
• Yellow skin or eyes, dark urine.

Precautions

• Allergy: do not take if you have reacted to any cephalosporin (cefixime, cephalexin, cefuroxime, ceftriaxone). Tell the doctor about any serious penicillin or Augmentin reaction.
• Kidney disease: dose interval needs adjusting.
• Past colitis or antibiotic diarrhoea: higher risk of C. difficile.
• Pregnancy: cephalosporins are among the safer antibiotic choices, still use only on a doctor’s advice.
• Breastfeeding: small amounts pass into milk, watch the baby for diarrhoea or thrush.

Interactions

• Antacids, omeprazole, famotidine, ranitidine: lower absorption, space 2 hours apart.
• Probenecid: raises cefpodoxime blood levels.
• Warfarin: may increase INR, blood thinner patients need a check during the course.
• Oral typhoid vaccine: keep 3 days apart.
• Lab tests: can give false positive urine glucose dipstick and Coombs test results.

Storage

Keep in the original box below 30°C, away from moisture and sunlight. Not in the bathroom. Out of reach of children. Do not use after expiry.
