Description

Cefobid® is a broad-spectrum, third-generation cephalosporin antibiotic containing Cefoperazone Sodium. It is used for serious bacterial infections requiring hospital-based intravenous or intramuscular therapy.

Key Benefits

  • Broad coverage against gram-positive and gram-negative bacteria

  • Effective in respiratory, urinary, abdominal, and skin infections

  • Reaches high concentrations in bile and body fluids

  • Suitable for monotherapy or combination therapy with aminoglycosides

Mechanism of Action

Cefoperazone works by binding to penicillin-binding proteins (PBPs), blocking bacterial cell wall synthesis and leading to cell death. It remains active against many resistant strains and achieves high bile concentrations, making it effective for biliary and intra-abdominal infections.

Indications

Used for infections caused by susceptible bacteria, including:

  • Respiratory tract infections (pneumonia, bronchitis)

  • Urinary tract infections

  • Intra-abdominal and biliary infections (peritonitis, cholecystitis)

  • Skin and soft tissue infections

  • Pelvic inflammatory disease and endometritis

  • Septicemia and other systemic bacterial infections

Dosage & Administration

  • Usual adult dose: 2–4 g per day, divided every 12 hours

  • Severe infections: up to 6–12 g per day may be used under specialist supervision

  • Administer via IV infusion (15–30 minutes) or IM injection

  • Dose adjustment required in hepatic or combined hepatic-renal impairment

Side Effects

Common: Mild diarrhea, nausea, injection site pain, transient liver enzyme elevation
Serious (rare): Hypersensitivity reactions, bleeding disorders, encephalopathy, or severe skin reactions (SJS/TEN).
Seek urgent care if you notice rash, fever, jaundice, or bleeding.

Warnings & Precautions

  • Allergy Risk: Avoid in patients allergic to cephalosporins or penicillins

  • Bleeding Risk: Monitor prothrombin time; vitamin K may be required

  • Alcohol Interaction: Avoid alcohol for 72 hours after injection (possible disulfiram-like reaction)

  • Hepatic Impairment: Dose reduction recommended

  • Prolonged Use: May cause secondary infections such as C. difficile-associated diarrhea

Storage

Store vials below 25°C, protected from light.
Use freshly prepared solutions; discard unused portions after 24 hours (room temperature) or 5 days (refrigerated).
