Vancomycin

Overview

Vancomycin (oral form) is used for bacterial infections inside the intestines, particularly Clostridium difficile-associated diarrhea and enterocolitis. It works locally in the gut and does not treat infections in other parts of the body, which is why injected Vancomycin cannot replace oral Vancomycin for colitis-related conditions.

Warnings

• Oral Vancomycin only works in the intestines. It will not treat infections elsewhere in the body.

• Do not use if you are allergic to Vancomycin.

• Inform your doctor if you have a history of bowel disease (IBD, Crohn’s, Ulcerative Colitis), kidney issues, hearing problems, or if you are already taking IV antibiotics.

• Use during pregnancy or breastfeeding should be medically supervised.

Dosage:

• Enterocolitis: 500 mg to 2 g per day divided into 3–4 doses, max 2 g/day

• Severe CDI (as per SHEA/IDSA guidelines): May include higher oral dose + rectal vancomycin + IV metronidazole, under hospital supervision

Side Effects

May include stomach pain, nausea, bitter taste, dizziness, hearing changes, or skin rash. Long-term or high doses may affect kidneys or hearing.

Precautions

• Complete the full course even if symptoms improve.

• Do not increase the dose — more is not more effective and may cause harm.

• If used long-term, regular medical tests may be required.

• Avoid mixing with alcohol or nephrotoxic drugs unless cleared by a physician.

Storage

• Capsules: Store at room temperature, away from moisture and heat.

• Liquid: Keep refrigerated, do not freeze, and discard after 14 days.
